Wet AMD pathway with direct referral to Royal Free Hospital

This pathway is open to all practitioners across NCL ( Barnet, Enfield, Haringey, Camden & Islington) who are referring suspect wet AMD patients.
The referral template and patient leaflet is below.
Please use this template which has been agreed with consultants at the Royal Free Hospital with significant LOC input. You can use your standard practice email for this with the patient's consent.
The fast track referral must be emailed to the following dedicated email:
rf-tr.amdurgentreferral@nhs.net
It is the practitioner's responsibility to inform the patient that if no fast track appointment has been made within 7 days they must get in touch with their referring practitioner for more advice.
The above contact details are only for suspect wet AMD cases and not for any other referrals.
